Community Health Hubs are physical sites in the community that provide a broad range of health & wellness resources & services that respond to community-voiced needs.

About

Community Health Hubs (CHH) are a collaboration between community-based organizations, community leaders, and institutions that share the collective goal of increasing access to health and social resources.

CHHs are responsive to the needs and culture of the community by taking direction from community champions, as well as investing in meaningful partnerships with community-led spaces and culturally specific programming that promotes community wellbeing.

CHH offerings often include preventive health screenings, food and nutrition resources, public benefits enrollment, activities promoting mental wellness and accessible movement, and more!
